Where was your print on demand Index made? I am thinking of ordering the print on demand books soon if there is a Thanksgivings sale or something.
As a curiosity I think I've read that "the ISBN of POD books are different for each book". I am confused whether that means the ISBN for POD copies is different from what the ISBN was for previous releases; or if each POD copy has a different ISBN (so no two POD copies would ever have the same ISBN). I would think ISBNs are consistent at least throughout all POD copies?

The print on demand books, come from HarperCollins via the tolkien.co.uk site. They do all the HOME editions and the Index in Hardback, the Index is below.

The Print on Demand books have a different ISBN to the original releases, but as an example all Print on Demand Book of Lost Tales 1 have an ISBN of 978-0-00-736525-8.
As far as I am aware they have never had any of the POD items on sale.

I need to get a hard cover index myself, I am waiting for the next sale announcement for tolkien.co.uk.
Also, and unusually for a POD, the HoME Index effectively has more than one state, due to that line number mess-up (on the copyright pg.) they made when they first printed it.

The number-line mess-up that Khamul mentioned. I was actually pleased with this cock-up as it clearly denotes the first print (as the first lot were printed as a batch, and were defective).

Yes. All the hardback PODs are printed and bound by "Martins the Printers", Berwick-upon-Tweed.
They are very well constructed solid hardbacks. The only comment would be they have flat spines, rather than curved.
I don't know who prints the paperback PODs.
