The Daily Mail have published an article about the 1921 Census and they include an entry on Tolkien.
Nothing else about Tolkien in the article so I have attached an image of the census entry, it saved me some time looking it up.

There're only 45 Tolkiens in UK census in 1921 (30 in 1901 and 5 in 1841), four are on this page, and Hilary makes the 5th 
No wonder Tolkien would say "The name is never to be met with – or hardly ever in directories or telephone books! and when I have come across it on occasion the owner has always turned out to be a fairly close relative."
