There are a couple of the Brian Alderson booklets on Abebooks for £55 and £75, and actually one on eBay right now (ends in 6 days) with 1 bid at £30.

There are a few different versions of the booklet (Top two UK, Bottom one US). See photo.

Looks like a 5th state. Jackets are gone unfortunately.
