Is it normally just stapled together photocopied pages?

And what's with the Tribute to CT?

Beyond Bree is sending a 2017 BB calendar signed by members (who met up with Nancy at a few conferences this year) to Christopher in thanks for all of his hard years of work. There is an invitation to send Nancy letters/comments/thoughts that will be included in the package if the reader was not able to sign the calendar directly.

BB (Nancy) has corresponded often with Christopher and met with him at least once that I know of, so this will not be unexpected or unappreciated by him.

On the photocopy question - I'll have to ask again (I suppose with the cost reduction in laser printing, these could all be printed rather than copied at this point, I don't know if she changed technology since the last time we talked and I can't tell the difference on cursory inspection). All physical copies are "bound" this way - many subscribers have opted with digital copies at this point, too.
